This market research survey is conducted by Service Management Group (SMG). So when you arrive at the survey site, you will see the words “Powered by SMG” at the bottom right of the screen.
You can take the JCP survey in either English or Spanish. To get started, you have to enter the survey access code, which is a 22-digit number found on the survey invitation at the bottom of your JCPenney receipt. To help you locate the code, there is a sample of the receipt on the site. This access code is only valid for 7 days. This means that you have to take the JCP survey within a week after your store visit. Please be aware that JCPenney associates are not eligible for this survey.

To get your JCPenney printable coupon, you have to do the survey on a computer which has a printer connected, because you have to print the coupon out directly after completing the survey.
The survey questions are easy. If you are familiar with customer satisfaction surveys, you will find them very easy to answer. Since it is not a long survey questionnaire, you can probably finish it within a couple of minutes. At the end, you have an option to provide your email address if you would like to receive JCPenney surveys, coupons or other promotional offers in the future.
